COVID-19 Office Procedures (Updated 7/15/2022)

Cozy Acupuncture will not be treating any known, or suspected, cases of Covid-19 in the office.
Following are our new office procedures for the meantime:

You need to download the above COVID-19 Informed Consent form, reviews, initials, signs, and scans (or takes a photo) and emails the form to [email protected]. If you can't email it to me, bring the form for your appointment. When you can't print the form after all, please inform me before you come to the office and you can fill out the form in the treatment room. (one time only)
In the case that you are a new patient, you need to download the above Consent to the acupuncture treatment form and reviews, signs and scans (or takes a photo) and emails it to [email protected]. If you can't email it to me, bring the form for your appointment. When you can't print the form after all, please inform me before you come to the office and you can fill out the form in the treatment room. (one time only)
The morning of your appointment you will receive a phone call (or text) to ask whether you have any suspected symptoms of Covid-19 or not. If you have, you need to reschedule or contact your doctor.
When you get to the door of the office, please go into the office and use hand sanitizer to clean your hands and wait for your appointment (Updated 7/15/2022).
It is recommended to wear a face covering if you feel safe by that. We will be wearing surgical masks anyway (Updated 7/15/2022).
You will be taken to the treatment room directly.
We will require “touch less” payments for all patients. You might be asked to put a credit card in the system to expedite that process at the end of the treatment. eGift Card, Paypal or Venmo is also welcome, if done at the time of service.
Follow up appointments can be scheduled prior to leaving the treatment room. Scheduling ahead of time on line, if you are able, will help to expedite our check out process.
Once your treatment is done, you will be taken to the office door.
